Inquiry Form

We want to ensure that we are the perfect fit for your child and family.  The inquiry form helps us get important information about your child to ensure that we are able to provide services.

Consultation 

After receiving your inquiry form our Director will reach out to schedule a free consultation.  This call is to help gather more information about your specific needs. During this call we will collect information about your insurance to help verify ABA benefits.

Insurance Verification

If you plan on using your insurance plan for services, we want to make sure that we're in network with your plan. We will get in touch with your insurance plan to verify ABA benefits!

Intake Packet

Once steps 1-3 have been completed you will be sent over an intake packet that can be completed electronically! This is an important step to schedule your assessment for services.

Assessment 

Once the intake packet has been completed, your provider will be in contact to schedule your assessment meeting. This meeting is where we will provide a full assessment through parent and direct interaction with your child. This assessment will be used to develop your child's personalized behavior plan.

Begin Services 

Once the assessment and your child's plan has been completed, we will submit to your insurance for authorization of services. Once approval is received, ABA services may begin!
